Performance & Development
The Salt Lake County Council passed a new Performance and Development policy in 2011. The new system links individual employee performance to the achievement of county and agency goals and also provides for quarterly conversations regarding the status of these goals. The focus is on accountability and results in addition to employee development.
Performance Appraisal forms that are effective 01.01.2012 should be submitted on the new 2012 5–point scale Appraisal Form. If you enter a value based on a 3-point rating in the system for 2012 it will be considered by the new system as a below standard rating. Please make sure that you are entering only ratings that are based on the 2012 5-point rating scale.

2012 Appraisal Form
- 5-Point Form. Due to popular demand the original 2012 5-Point Form, which was a fillable pdf, has been replaced with this shorter excel form which will automatically calculate scores. As you complete the appraisal be sure the goal weights total 100. This form should be used for all evaluations effective 01.01.2012. (Points based on 1, 2, 3, 4, 5)
